Consent to the Processing of Personal Data

I, the subject of personal data, in accordance with the Law of Ukraine “On the Protection of Personal Data” (hereinafter referred to as the Law), give my consent to LLC Furniture Factory LATO (hereinafter referred to as the Operator) for the processing of personal data provided by me in the web chat form and/or in the callback request form on the website owned by the Operator.

1. The personal data I provide includes: full name, email address, and phone number.
2. The purposes of processing my personal data are to ensure the exchange of short text messages in online dialogue mode and to enable the functionality of the callback service.
3. Consent is granted for the following actions (operations) with the personal data specified in this consent: collection, systematization, accumulation, storage, clarification (updating, modification), use, transfer (provision, access), blocking, deletion, destruction—carried out both with the use of automation tools (automated processing) and without the use of such tools (non-automated processing).
4. I understand and agree that the provision to the Operator of any information about myself that is not contact information and not related to the purposes of this consent, as well as providing information related to state, banking, and/or commercial secrets, information about racial and/or national origin, political views, religious or philosophical beliefs, health status, or intimate life is prohibited.
5. If I decide to provide the Operator with any information (data), I undertake to provide exclusively accurate and up-to-date information and have no right to mislead the Operator regarding my identity or provide false or inaccurate information about myself.
6. I understand and agree that the Operator does not verify the accuracy of the personal data provided by me and has no ability to assess my legal capacity, assuming that I provide accurate personal data and maintain such data in an up-to-date state.
7. This consent is valid until the purposes of processing are achieved or in case of loss of necessity to achieve these purposes, unless otherwise provided by the laws of Ukraine.
8. The consent may be revoked by me at any time based on my written statement.
